The patient flyer from regular dose Naltrexone warns about taking Naltrexone when the patient has been on regular opioid dosing. Up to a two week gap is required before starting.

"To prevent occurrence of precipitated withdrawal in patients dependent on opioids, or exacerbation of a pre-existing subclinical withdrawal syndrome, opioid-dependent patients, including those being treated for alcohol dependence, should be opioid-free (including tramadol) before starting REVIA treatment. An opioid-free interval of a minimum of 7 to 10 days is recommended for patients previously dependent on shortacting opioids. Patients transitioning from buprenorphine or methadone may be vulnerable to precipitation of withdrawal symptoms for as long as two weeks."

As a safety measure before starting a naloxone (N.B. NOT Naltrexone) challenge test is administered. If interested this would be a relatively low risk way of determining your risk of precipitated withdrawal when using LDN.

Wow! Thank you so much! That was a total duh brain mis-fire on my part jumbling the profile name. All fixed now. Bravo for spotting that!!!

You're very welcome and we appreciate the appreciation! :)

Using Naltrexone for conditions other than addiction is an "off-label" use. This is common practice that your doctor should be familiar with:

Taking it with food may help with nausea. Filler is an issue for some -
Most reports of filler/ingredient trouble are with Avicel (Microcrystaline cellulose/MCC/cellulose) even though it may be tolerated in other meds/supplements.

Finding your optimal dose (which may NOT be a higher dose like 4.5mg)...

At some point during the titration up process you may feel you were doing better on the previous dose. If you still feel that way after giving your system two weeks to adapt, then you may have found your optimal dose. Then skip a dose or two and drop back to the previous level. May occasionally want to test an increase to see if there are additional benefits, but consider finding your optimal dose whatever it is as a "win."

It would be an experiment, but I would bet tempted to try a slight reduction in your thyroid meds and see what you notice. LDN can improve thyroid function and it may be that the way you are feeling is a better indicator than the tests.

Some have to start LDN with a lower than typical dose.

Taking any vitamins/supplements? They can affect urine smell (e.g. vit B-1). See if Google thinks it could be a result of anything you take. I don't recall it being mentioned as a LDN side effect.

Can't say if that would be right for you. Optimal dosing and scheduling varies with the person. A person dosing every 24hr will have their highest minimal blood level raised over the course of 12 days from starting. This gives some logic to the usual two week guidance.

Sublingual drops -- formulas vary but tend to be simpler/cleaner vs. other liquid versions. Example - Dickson's formula: Pure Naltrexone Powder, Water, Glycerol. Check your pharmacy's ingredients. Good for those with GI issues. Dose with the included dropper. If you are very sensitive in terms of GI issues ask your pharmacist how long you would need to hold the dose in your mouth to have it adequately absorbed before spitting out what's left so that less of the dose goes to your stomach. While holding if you have an irresistible urge to swallow tilt the head forward so the dose stays mainly in the mouth. Let us know what your pharmacist says.

Other possibility -

OK to try it. Optimal dosing varies widely with the individual and condition.

Some find that LDN can help with fatigue. Initial reactions often settle within two weeks. If not you may want to skip a day or two and go to a lower dose.

I would try to get sublingual rather than "liquid" as the latter tends to have a lot more ingredients.
