You may do better absorbing a progesterone oral troche than using the capsules. You may not be able to absorb them well if you have gut health issues. It needs absorption with a fat like Greek yogurt or peanut butter and maybe you’re also sensitive to the peanut or soy oils the caps come in. While you’re already off kilter from not absorbing the topical progesterone it’s tough to figure it out. Patience is key.

FWIW I was on 250 mg progesterone and opened a 25mg cap to divide it and I’m bumping up using 12.5mg (approximately) slowly and I still have side effects but they’re manageable/tolerable. I know I could not handle a 100mg bump all at once. No way.

I would consider discussing with your HRT doctor about an oral troche and taking that along with with a fat like full fat Greek yogurt. Ask the doctor if you should put the troche in the mucus membrane between teeth and cheek and let it sit a few seconds. It’s possible you may not have optimal stomach absorption with the compounded oral powder filled capsule. Women who have the oil filled caps discuss biting down and letting the gel swish in the mouth before swallowing for better absorption.

ETA - FWIW I had this same experience with compounded progesterone caps and transdermal. I also had to bump the dose slowly until bleeding stopped. Easy enough to do with compound capsule.

Is it possible you’re not really absorbing the estrogen from the patch? Perhaps you would be a gel or compound estrogen candidate, or you just maybe don’t absorb well transdermally and you’d do better with oral or injectable estrogen if your doctor clears you for that.

I wish more docs would recommend slowly titrating doses. This is so unfortunate and I wonder if you tried slowly titrating with compounded progesterone in small 10mg doses you might adjust better.

That can be histamine intolerance, especially if you’re having left overs or meats that have been aged. Any canned fish can also cause this issue.

There may be more to this than your doctor is telling you. It may be an absorption issue, especially if you have gut issues. If it’s not long acting, you could try biting down on a capsule and putting some of the contents between gum and cheek, or swish around the mouth. It’s good to mix with a fat like Greek yogurt or peanut butter so it passes through the liver. The oral mucosal absorption could give better results for you, or you might do better with vaginal or rectal suppositories. There are still options including what your doctor mentioned.

Maybe you can try an online provider service like Midi. I’m not recommending them as such, but there are many now.

There’s another trick. If you are not taking long acting progesterone and you’re taking the oil based capsules, chomp on them before swallowing and let the oil get in between your teeth and cheeks. Progesterone is best absorbed by a mucus membrane.

I’m opening my powdered compound caps and dissolving them in coconut oil and refrigerating them to make a troche. Then I loosen the troche with some warm water on the underside of the containing TBS and put the troche between my teeth/gums and cheek. Take with full fat Greek yogurt.
