I’m literally a hematologist and I don’t see anything wrongdoing from the reports that we’ve heard so far. People need to calm down.
DancingWithDragons
There’s no malpractice here. You can have an SVT and still be ok to play football. The SVT can also develop into a DVT after the fact.
DancingWithDragons
Stupid take. Factor xii deficiency would cause bleeding not clotting.
No contraindications to sleeping with a clot. Also no contraindications to playing with an SVT assuming it wasn’t close to a deep vein.
DancingWithDragons
I’m a hematologist. The amount of stupid takes being thrown around are insane. This is one of the only good comment I’ve seen so far.

I’m all for changing the program to actually help safety net hospitals. But I’m not ok with keeping the current status quo where non-safety net hospitals are benefiting immensely.
And you should absolutely care that healthcare systems don’t monopolize care in an area because that leads to increase costs for everyone.
DancingWithDragons
Some are, but hospital systems use 340b programs in low socioeconomic status areas to open a ‘branch’ in a high socioeconomic status area and get the same discounts despite having a much wealthier patient population. There are so many very easily exploitable loopholes in this program. 340b pricing should really have been tied to individual patients rather than hospitals.
DancingWithDragons
Highly support this move. This is another way big hospital systems are able to run independent practices out of business, because in addition to facility fees, they get to make 40-50% profit on drugs while independent practices are making 6%.
And because pharma companies are legally required to give these big hospital systems discounts, they raise the price for everyone else to recoup their losses. Result is high costs for everyone, massive profits for 340b systems, and the patients end up paying more either upfront or with increased insurance premiums.
DancingWithDragons
Good. 340b was a good thought but was used by hospitals to get massive drug reimbursements from wealthy populations it was never meant to serve.
DancingWithDragons
Yup, like 40% benign heme since I’m still building my practice. Remaining 60% is all a variety of onc and malignant heme. If I had 15 malignancy patients I’d imagine my prep would be more like 1.5h

New Onc attending. About 1 hour for ~15 patients. More if complex or lots of new pts.

Shortage of all platinums according to our pharmacists here in the Midwest. Still able to get what we need for now.

Train AI to do your job for $200-400/hr. No thanks.

Cobra, which is a continuation of your previous coverage. It was about $1400/mo for me and my wife.

I’ve done probably over 100 at this point. You get good at the usual ones after like 20-40. However no matter how good you are, you’ll have the odd patient with technically difficult anatomy, high bmi, extremely soft/hard bones, and dry taps. That’s what IR is for. Patients honestly prefer it that way since they get sedation instead of just local.

The more I look at this the shittier of a deal this is. Dealers are begging to sell the new x3 cause it’s a shitty design and bad interior quality. This is a 1 year old loaner and it’s only discounted about 16% and with fees that make that even smaller. You can get 10% off a new x3 with no fees all day long at a lot of dealerships right now.
Check out leasehackr.com and do some more research.
DancingWithDragons
What is the capitalized registration and fees and the dealer fee? That’s about $3k in fees which seem to just be pure profit for the dealer. You also have a 1k protection plan (not sure if you wanted that). Seems like this dealer is giving you a massive upfront discount and then making a good portion back with fees.

Im heme/onc private practice. Your numbers for post-partnership are likely low unless your volumes are low or your practice’s overhead is high.
I’m in a HCOL place and for 15-20 pts, 4 day work week and 8 weeks vacation the average is $700-800k. If the partners are doing 5 day work weeks and 4 weeks off, they should easily be at $1m.

To add to this, even without 340b, having a hem/onc program leads to so much revenue for a hospital system: infusion, labs, imaging, radiation, surgeries. Heme/onc salaries are subsidized by the downstream revenue they bring in, not just their direct collections.

I bought a Uniqlo down parka in 2019 and it’s still in amazing condition. Insanely warm too. Patagonia will be better obviously but for the value, the Uniqlo one is the winner.
