This is cool, I had no idea this was released as a booklet. If I had to guess, I'd say it may have been part of the Goosebumps Fan Club merch. There may be a pic of it in one of the printed ads in the back of a goosebumps book from that time.

I really like the WC Jameson Buried treasures of ______ series. He has shirt stories about treasure collected from folklore and arranged by region of the US. They are cool if you like treasure hunting or just Western short stories.
