yeah i unfortunately am guilty of ordering unnecessary things just to shut down back and forth secure chats with RNs

Some of the hospitals thankfully have a protocol that allows our RTs to de-escalate frequency down and to PRN as their breathing improves.

The problems being solved right now are fields/turing worthy and with very little expert input. Humans are barely able to keep up with the verification process because these problems now being solved with increasingly frequency. You can just listen to testimony from actual mathematicians who are having an existential crisis about their careers. One of the reasons Jacob Tsimmerman who won the fields medal _this year_ left academia.

words have lost meaning. everything is slavery and genocide

gmdmd

Mathematicians are in shambles rn... lay person like me can prompt out their phd thesis. I find it quite sad.

gmdmd

Surprised we haven't seen him on RAF but he's probably building up his cooking channel...

gmdmd

would agree if on low-normal. i keep getting mine checked hoping I can hop on the sauce but still upper normal :'(

gmdmd

what are you some sort of rocket surgeon..

gmdmd

what’s your take home?

gmdmd

Unfortunately I went to a couple of apixaban sushi dinners... but all of the heme docs and pharmacists say it's the best!

gmdmd

I was doing that before but now I set my muse agent to reload $7 a month...

gmdmd

he lost a bit of aura with the complaining tbh...

I'm pretty sure I'm in the top quintile of all docs in terms of having these conversations and general stewardship.

But I'm not going to virtue signal on reddit that this stuff isn't insanely common or that I haven't taken the easy road. We've all had patients that are flagged by risk management, lawsuit against the hospital ongoing, living in the hospital for 3+ months in a vegetative state and multiple palliative care teams have failed to make progress. You check your box, see the tortured patient and move on rather than engage a crazy family.

You can send in a mid-level. They already do this in the ER. An AI (could be specialized for history collection) can remain on standby as they collect history and prompt the mid-level to ask additional questions so you don't have to bounce in and out of the room repeatedly. Once you have objective information collected for the day a different AI can synthesize and come up with an assessment and plan.

We haven't done it yet because it takes time to build and red tape in the US. They are starting to roll out AI clinics in China...

gmdmd

not hiding behind anything i live in the real world and we see a lot of unnecessary shit and everyone is guilty of it

sometimes when you have 20+ patients to see it's easier to torture grandma than get yelled at by a crazy family so you can move onto caring for other sick patients

gmdmd

I mean we do a lot of futile stuff to meemaws and others without consideration of dignity to appease families. Nothing shocking really.

gmdmd

haha yup. residents are way underpaid and under-appreciated. small gestures like this go a long way, whether nefarious or not.

gmdmd

It's like taking a sammie from pharma though (I'm guilty of this).. you think you're above the influence but once you eat a bite you're subconsciously tainted.

gmdmd

This round and him standing over Islam in their first fight... peak Volk, such a badass.

gmdmd

people are desperate to prove they're not a casual, they can't admit shit is boring

gmdmd

if they weren't going to respect the gentleman's agreement the ref should be encouraged them to stand it up after 1-2 minutes of no action on the ground.

gmdmd

Also have to imagine it's hard to find similarly handicapped practice partners to wrestle. It's got to be tricky going up against that for the first time at a high stakes environment.

gmdmd

You could also look into /r/DeadBedrooms it's more common than you'd think. not necessarily anyone's fault.

gmdmd

every 2 weeks you can buy $200 in uber/uber eats cards from costco online for $160.

gmdmd

LOL

gmdmd

yes also it spares the poor locals trying to get to work from being crammed like sardines with all of the tourists!
