I've reported places to the NJ DOH. You can do it directly from their website. I believe their forms require you to include your information as the person making the complaint, but from there it's confidential, supposed to be in theory.

I even got a letter, that I kept and have floating around somewhere, which states something along the lines of, you reported an issue. An issue was found and actions have been taken.

Didn't tell me exactly what was found or the actions, but I was like, f ya.
