I am a COTA working in EI for the last 5 years. It is pretty common for last minute cancels or no shows. Is there a service coordinator that helps manage the case? It might be good to reach out to other team members.
Also as an OTA I assume there is an OTR doing the initial consults. Maybe reaching out to them for some support/guidance.
